I would say thumb up rather than thumb out. Also my dad accidentally put his hand through a glass door and I don't think he had any lasting damage of any kind. We mostly think of it as a funny story because he was so mad at his boss (on behalf of my mom, who also worked there) that he didn't even notice what had happened until the boss was like "your arm is soaked in blood, please let me call 911 for you"
